Message from Nurse Sandy Bacerdo-

Welcome back all returning families and welcome to all our new Bellevue High School families!

My name is Sandra Bacerdo and I am the School Nurse at Bellevue High School. This the start of my 4th year at Bellevue High, my 13th year in the Bellevue School District and my 37th year as a Registered Nurse. I am looking forward to working with you and your students to meet their health and safety needs.

Due to the limited access to the building at this time the best way to reach is by email at I am also checking phone messages at 425-456-7005. My work hours at Bellevue High School are Monday -Thursday 7:45 AM -3:30 PM and every other Friday.

This message is for our returning students and new students to BHS with individual health plans, medication authorizations and/or other medical treatment plans.  For returning families, this message is a follow-up to an email you received from me in June which contained Individual Health Plans, Medication Authorization form, and/or treatment forms. For new families, you likely received an email upon registration containing the same forms. If your student has a life-threatening condition (food allergy, seizures, diabetes, etc.) they will require an Individual Health Plan.

If your student requires medication (whether prescription, over the counter, or herbal/naturopathic) or treatments for ANY condition while at school, this will require a Medication Authorization form or Treatment Authorization form. These forms require a signature from a licensed health care provider. In terms of health and safety these must be in place before attending any on campus activities which also includes co-ed conditioning, sports, clubs or activities. This pertains to Running Start students as well.

 While we are starting the year off remotely, I am still working on gathering health information and necessary forms in anticipation of a hybrid model of school. The best way to return these forms to me is by email. If this is not an option for you, please fax these to 425-456-7174 or mail to the school.
Washington State law requires that the necessary health care provider orders, medications, individual health plans, and other orders must be in place BEFORE students may attend school or on campus activities. This law can be referenced at:

While the start to school was quite different, we are still following state laws and guidelines as it pertains to the health and safety of our students. On August 1st changes to Washington State immunizations rules went into effect. The three main changes as they pertain to schools include:

  • Medically verified immunization records
  • Clarifies conditional status
  • Changes Tdap immunization requirement

Requirements for medically verified immunizations include:

  • A Certificate of Immunization Status (CIS) printed from the Immunization Information System
  • A physical copy of the CIS form with a healthcare provider signature
  • A physical copy of the CIS with attached medical immunization records from a healthcare provider.
  • A CIS printed from My Immunization Records (MYIR)

We encourage ALL BSD families to get the yearly flu shot. The annual flu shot helps decrease the severity/complications of the flu. If people get their flu shots, fewer people come down with the flu and come into healthcare providers and clinics (hospitals) with nonspecific symptoms such as fever and cough, which overlap with symptoms of COVID19.  Having fewer flu patients will make it easier to find the those with COVID-19.
