What is Health Check?

Health Check is a web-based form that asks people to answer whether they have signs and symptoms of COVID-19, have tested positive for COVID-19 in the past 10 days, or have been in close contact with someone who tested positive for COVID-19 in the past 14 days.

Why is BSD doing this?

A daily health check is one of the ways we are working to minimize the spread of COVID-19 in our learning community. The health check will help us keep people who have symptoms of COVID-19 or have been exposed to COVID-19 from coming into our schools and facilities and spreading the virus.

Who is expected to participate?

Staff, students and substitutes entering our buildings will be asked to complete a health check every day before they come into a school building or other district facility. Essential visitors, including student teachers and agency staff/contractors will complete their health check using the LobbyGuard visitor check-in system.

How to Complete Your Health Check


How do I access the daily health check?

You may access Health Check on any device with internet access via the https://healthcheck.bsd405.org website. We recommend completing it no more than four hours prior to arriving onsite. Staff without access to the website may complete their health check using LobbyGuard. Students and families without access to the website may use the computer kiosk at school or may ask a staff member to complete it on their behalf.

I received a QR code to access the daily health check. I don’t know what to do with it.

Scan the code with your smartphone. Open your camera, point it at the code and click the link that appears on your phone. Most phones should not require a code reader app.

iOS

  1. Open your camera and point it at the code for 2-3 seconds.
  2. A notification will appear.
  3. Click the link that appears.

Android

  1. Open your camera and point it at the code for 2-3 seconds.
  2. When scanning is enabled, a notification will appear.
  3. If nothing happens, you may have to go to your Settings app and enable QR Code scanning. Some phones provide the Bixby Vision service which allows your camera to read QR codes. You may need to sign-in or accept the terms and conditions of the service.

If QR Code isn’t an option in your Settings, your device unfortunately can’t scan codes natively. This means you’ll have to download a third-party QR code reader app. See recommendations (not endorsed by BSD).

Do I need to download an app to access the daily health check?

No, the daily health check does not require you to download an app. It is accessible on any device by visiting the https://healthcheck.bsd405.org website.

This is a new process for me. Can I receive reminder text messages, emails or phones calls?

We do not have a reminder service. You may set your own reminders on your smartphone or use other techniques for remembering daily tasks.

Can my student complete the daily health check on their own?

Parents/guardians should complete daily health checks for students in grades K-5. Students in grades 6-12 may complete the daily health check on their own.

How do I save the website on my phone for easy daily access?

You may bookmark or favorite the https://healthcheck.bsd405.org website like you would for any other website you wish to refer to later.

Can I access Health Check like an app from the home screen on my mobile device?

Yes. An icon can be added to your home screen so you can quickly access the website. Learn how (PDF).


Issues and Troubleshooting


I tried scanning the QR code on my phone but can’t get it to work. What should I do?

Some newer model phones can scan QR codes with their camera, while for others, you may need a code reader app. You can access Health Check by navigating to https://healthcheck.bsd405.org using the browser on any device with internet access.

Does the daily health check allow me to access, edit, share or delete my data once it has been submitted?

No. Once you have completed the daily health check, you cannot access, edit, share or delete your data.

If I made an error and need to change my answers, what should I do?

You may resubmit your daily health check.

I am having issues with the daily Health Check website, who can I contact for help?

You may contact the Help Desk at 425-456-4321 or email help@bsd405.org.

I don’t have internet to access the Health Check website. What are my other options?

Students and families: You may use the computer kiosk at school, or you may ask a staff member to complete it on your behalf. Your alternate option is to turn in paper forms to your school. Paper forms will be available.

Staff: You may complete your daily health check using LobbyGuard. Your alternate option is to turn in paper forms to your supervisor. Paper forms will be available.

Staff will record the data from paper forms into the Health Check system.

I prefer not to enter my information online. What are my other options?

If needed, the Health Check form (PDF) is available to download and print. Forms will also be available at the front office at schools, and supervisors can provide staff with paper forms if needed.

Students and families: Completed paper forms may be turned in to your school.

Staff: Completed paper forms may be turned in to your supervisor.

Staff will record the data from paper forms into the Health Check system.

Is there a preferred browser for the daily Health Check website?

No. The website is accessible using any browser on any device with internet access. Contact the Help Desk at 425-456-4321 or email help@bsd405.org if you experience any issues.


Compliance


What if I don’t complete the daily health check?

The daily health check is a critical step in our efforts to minimize the spread of COVID-19 in our learning community.

Students and families: For K-5 students, a staff member will call parents/guardians to remind them to submit their child’s health check. For 6-12 students, a staff member will contact them and remind them to submit their health check.

Staff: If you forget to complete the daily health check, someone from the school building or district facility will contact you to remind you to complete it.

Do I have to complete the daily health check if I am not going to a BSD school or district facility?

No, if you will not be going to a BSD school or district facility, you do not need to complete the daily health check. The daily health check is to be completed by those planning to visit a building in person.


For Students


If I’m learning virtually, do I need to submit the daily health check?

No, you do not need to submit a daily health check on days when you are not physically present on campus. Virtual students will need to submit a daily health check if they will be physically going to a school building for athletics or activities.

Why are virtual students not expected to submit daily health checks?

We are using daily health checks to help minimize the spread of COVID-19 in our buildings, so students who are not coming in do not need to use Health Check. Any virtual student who may be isolating or quarantining at home for COVID-19 symptoms or COVID-19 exposures should reach out to their teacher or another trusted staff member in their building if they need help or supports related to COVID-19. If a virtual student shows symptoms of COVID-19 or has been exposed to someone who tested positive for COVID-19, they should talk to their healthcare provider.

If I’m learning at school, do I need to submit the daily health check?

Yes, if you are learning at school, you (grades 6-12) or your parent/guardian (grades K-5) needs to submit the daily health check.

If I’m a hybrid student, do I need to submit the daily health check on days I am at home?

If you are a hybrid student, you only need to submit the health check on days you physically come into a school building, or participate in an activity or athletics in person at school or another district facility.

I do not know my student ID. What should I do?

Students may login to StudentVUE and parents/guardians may login to ParentVUE to retrieve student IDs. Your student ID is the same number you use to purchase breakfast or lunch. If you don’t have a StudentVUE or ParentVUE account, or experience any issues, contact the Help Desk at 425-456-4321 or email help@bsd405.org.


For Staff

I am working remotely. Do I need to submit the daily health check?

No. You do not need to complete a daily health check on days when you are not physically present on campus or at another district facility.

Why are remote employees not expected to submit daily health checks?

We are using daily health checks to help minimize the spread of COVID-19 in our buildings, so employees who are not coming in do not need to use Health Check. Any remote employee who may be isolating or quarantining at home for COVID-19 symptoms or COVID-19 exposures should reach out to their supervisor if they need help or supports related to COVID-19.  If an employee working 100% remote shows symptoms of COVID-19 or has been exposed to someone who tested positive for COVID-19, they should talk to their healthcare provider.

Which employee groups are expected to participate in the daily health check?

Any employee who works in person in the Bellevue School District must use Health Check every day that they come in to work, even if they are working alone or outside.

How does this affect contractors, agency workers, vendors, student teachers and other visitors who are considered essential district staff?

Approved essential staff will submit their daily health checks using the LobbyGuard visitor check-in system. If a LobbyGuard unit is not available at your building, paper Health Check forms (PDF) may be turned in to your supervisor.

I do not know my employee ID. What should I do?

My primary location listed in Health Check is incorrect. What should I do?

You may contact HR at 425-456-4040 or email hr.info@bsd405.org and they will update your primary location. For your daily health check, check the box for Alternate/Additional Location and select your work location(s) in the drop-down menu.

I am a substitute and my alternate location says Substitute Contractor Special. This is not a location. What should I do?

Check the box for Alternate/Additional Location and select your work location(s) in the drop-down menu.


Transparency


What information does BSD collect as part of the daily health check?

Any reported symptoms of COVID-19 (e.g. fever, cough, shortness of breath, etc), possible exposure history and testing.

What happens to the information I submit?

Data collected will be stored in a secure database until data retention is met.

How does BSD use my daily health check data?

BSD will use the data to ensure student and staff wellness. It will only be shared internally with a limited number of staff whose role is to assist in risk mitigation efforts and contact tracing outreach. We will use the data to determine who is permitted to be on campus and when staff and students can return after illness. It is also used to notify individuals who have been in close contact with individuals diagnosed with COVID-19 so that they can take additional precautions. Staff are required to maintain its confidentiality.

Does BSD share my daily health check data with any third parties?

Data submitted as part of any daily health check will not be shared with vendors. We created the website internally and do not depend on an external entity for Health Check website development, support, operations or maintenance.

COVID-19 is a nationally notifiable disease, and we are required by law to notify our local public health jurisdiction (Public Health – Seattle & King County) of any positive cases and close contacts in our school buildings. If you test positive for COVID-19, we will notify Public Health and conduct a contact tracing interview with you to determine who you may have been in close contact with in our school buildings during your infectious period. Your personal health information will be kept confidential. We must notify any close contacts we discover during our interview with you that they have been exposed and must quarantine at home, but we will not let them know who they were exposed to.

Does BSD sell my daily health check data?

No. We will not use the data for anything other than for its intended purpose.

What happens to my daily health check data when I am no longer enrolled or employed at BSD?

When you are no longer affiliated with BSD as a student or employee, BSD automatically deactivates your account. Daily health check data that hasn’t already been purged at the time of account deactivation will be purged in accordance with BSD’s normal practice.


Privacy


Who do I contact if I have more questions about my daily health check data and how BSD is maintaining its privacy?

You may contact the Help Desk at 425-456-4321 or email help@bsd405.org. BSD will use reasonable efforts to address your concerns.


Wellness


I don’t have a thermometer to check my temperature at home. Will BSD provide one to students, families and staff?

We do not have thermometers to provide students, family and staff at this time. Please monitor yourself and your children for other symptoms of fever, including sweating, chills and shivering, headache, muscle and body aches, lethargy and general weakness, irritability and fussiness, feeling faint/dizzy, loss of appetite and eye pain.

The daily Health Check told me to self-isolate and stay home. Was the attendance specialist at school automatically notified? Was my supervisor notified?

Students: If you need to self-isolate and stay home, you should follow your normal school protocols to report an absence. The COVID-19 attestation specialist at your school will be notified. You will be contacted by a trained staff member to implement contact tracing if needed.

Staff: If you need to self-isolate and stay home, you need to alert your supervisor. Your supervisor will initiate contact tracing if needed and work with you about connecting with HR so you can take leave if needed.

If I need to be tested, where can I go?

Public Health – Seattle & King County has a listing of COVID-19 testing locations: https://www.kingcounty.gov/depts/health/covid-19/testing.aspx. Also, your health care provider may be able to recommend a testing location.

What if I have an existing condition with the similar symptoms (e.g. asthma, seasonal allergies) listed in Health Check?

If you have an existing condition with symptoms similar to COVID-19 symptoms, you should speak with your healthcare provider who may provide you with a letter indicating that your symptoms are unrelated to COVID-19. Please provide this letter to your school nurse or supervisor. When we return to broad in-person school, families may reach out to their school nurse with questions about whether their child meets the criteria to come to school. They can also call the BSD Nurse Hotline at 425-456-4444.

What guidelines does BSD follow to define what COVID-19 symptoms are or what is considered an exposure?

The Health Check questions are based on public health guidance from Public Health – Seattle & King County, Washington State Department of Health, and the Centers for Disease Control. The questions are subject to change based on changing guidance.
