Step 1: Prepare Your Enrollment Documents
During enrollment, you will need to upload the following documents for each student. Please make sure all the documents are saved in the same folder (i.e., the desktop of your computer.) Photographs from a smartphone are accepted if the image is readable.

These documents are required for enrollment if they apply to your student:
- Age verification (for kindergarten and 1st grade only) generally families provide a copy of the student’s birth certificate or passport. Click here for more options.
- Unofficial Transcript (for 9th – 12th grade)
- Last report card from previous school (if applicable)

These documents are important to ensure that your school knows if your student has specific needs:
- For students receiving Special Education services:
- Most recent IEP (Individualized Education Program) and current evaluation
- Current 504 plan, the most recent Individual Health Plan or doctor’s treatment orders
- Medical Authorization Form (PDF) for students who need medication at school
- Medical History & Exam (PDF) for high school athletes
- Support for Native American Students (Title VI Program) Form (PDF) for students who select Native American Indian in the Race and Ethnicity section of the enrollment form
